All non-suspicious deaths, including those that occur in a hospice setting, should be documented as follows:
- CAD Call notes indicating the following:
- Name, Age, Medical History
- Approximate time of death and by whom it was made
- Death certificate-signing doctors name and phone number
- Mortuary taking possession of the body
- Next of kin (if available)
- Corner notification, if a doctor is unable to sign
- Sergeant who was notified
- CAD clearing code - P300
- The Commanders log entry -completed by a sergeant, with the same information.
